Free policy acknowledgement form

When you introduce or update a policy, a handbook or a code of conduct, it helps to have each person confirm that they received it and read it. This starter form asks them to tick three confirmations and sign. With bulk send on the Solo and Teams plans you can send it to everyone from a spreadsheet and track who has signed.

Who signs

The person signing, on their own phone or computer from a link in an email. They don’t need an account.

What you fill in before sending

  • Your organisation’s name
  • The name of the policy or document
  • Its version or date, if you like
  • Where to find it, for example attached or a web address

What the signer adds

Apart from their signature, their name (filled in for them) and the date:

  • Three ticks: they received and read it, they understand it and will follow it, and they know they can ask questions

What it covers

  • What the person confirms
  • About this form: it records receipt and does not change the policy or any contract

When to use it

  • A new or updated staff handbook, health and safety policy, data protection policy or code of conduct
  • Volunteers, members or contractors who need to confirm they have read your rules
  • Sending the same form to many people at once with bulk send

When it’s not the right form

  • Changing someone’s contract or terms of employment, which needs proper agreement and may need advice
  • Policies people must agree to in law in a particular way

How to get it signed
